I voluntarily consent to a relaxation therapy session at Hilo Energy Therapy, LLC. I agree to comply with all recommendations to the best of my ability in order to receive maximum benefit.

I am responsible for the decision to seek this type of relaxation therapy program that could include improvement of the physical, psychological/emotional and spiritual aspects of my life. I recognize that the Hilo Energy Therapy staff do not treat specific disease or illness and are not presently licensed, certified or registered by the state of Hawaii as healthcare professionals. However, all staff have been trained as technicians for administering sessions for clients. I recognize the possibility that this program may not prove successful in accomplishing the results I expect or hope for.

Not all traditional medical practitioners recognize this approach to health. Clients should discuss any concerns with their medical professional.

I understand that any therapy sessions or suggestions to enhance overall body performance represent our best guess based upon past experience and are not meant to be considered as a prescription.

Hilo Energy Therapy reserves the right to deny treatment if it is not deemed to be in the best interests of either client or staff.

Finally, I waive any claims and demands against Hilo Energy Therapy and staff from any and all liability relating to the therapy sessions.
